What is Skilled Nursing?

A Skilled Nursing Facility (SNF) is a licensed Medicare and Medicaid facility with the main service offering being short term rehabilitation.

As a skilled nursing facility, we provide skilled nursing care and/or rehabilitation services to empower people to achieve their highest level of recovery and participation in life through excellence in rehabilitation. Our goal is to help injured, sick, or disabled individuals to get back on their feet and back home.

Our skilled nursing facility’s vision is to provide world class rehabilitation solutions advancing the health and well-being of our communities.

Two Types of Skilled Nursing Care

Short-term care

Our main service offering is short term rehabilitation. If you have experienced a stroke, an orthopedic surgery, fall, or need help with a medically complex diagnosis we are here for you.

Long-term care

We also offer long-term nursing care and rehabilitation. Our SNFs offer hospice services for end of life and support to our residents and family members.

Skilled Nursing Services

As a skilled nursing facility, we provide 24/7 care. We are staffed with trained medical professionals, such as in-house physicians, licensed nurses, physical, occupational and speech therapist and more.

Skilled Nursing

Our SNF provides extensive care services, such as intravenous feedings, blood pressure monitoring, medication injections, post-surgical care, wound care, continence care, diabetic and oxygen management.

Rehabilitation

Our skilled nursing facilities have designated gyms to provide physical, occupational, and speech therapy. We have world class equipment and technology to help you achieve your highest level of recovery. Some of our unique equipment includes Functional Electrical Stimulation Bikes, Synchrony Biofeedback for swallowing disorders, Lite Gait body weight support system, and Bioness for upper and lower extremity.

Healthcare team

Our healthcare team includes registered nurses, Licensed Practical Nurses, licensed therapists, CNAs, Case managers, Activities staff, Minimum Data Set coordinators, Physicians, Nurse Practitioners, wound care nurses and Dieticians.

Services & amenities

Our facilities offer 24 hour care, resident and family care meetings, daily activities, food service, daily housekeeping, laundry, therapy programs, social services, and wound care services.

Our programs promote social and emotional well-being, such as our Music Therapy program, designed to elicit different responses from the brain using music, and our Low Vision Program, intended to help patients to make the most of their remaining vision by continuing to participate in daily activities and maintaining independence.

Skilled nursing facility vs. assisted living

Skilled nursing facilities are generally designed for seniors who require 24-hour medical supervision due to physical or mental conditions that leave them unable to care for themselves.

Assistive Living communities are a unique option for older adults who are mostly independent but require some assistance with day to day living.

Bartram Crossing

Bartram Crossing is a skilled nursing facility on our beautiful 115-acre campus in Bartram Park, providing short and long-term rehabilitation and nursing care. Guests of Bartram Crossing can expect three individual neighborhoods with private rooms and common areas, an expansive state-of-the art therapy gym, courtyards, peaceful walking paths and lakes.

Our care teams are committed to ensuring our guests receive both the highest quality care and best possible experience. Bartram Crossing is staffed by comprehensive care teams that include a physician specializing in rehabilitation, nursing staff trained in rehabilitation, physical therapists, occupational therapists, speech therapists, dieticians, recreational therapists and psychologists.

University Crossing

University Crossing skilled nursing facility is located on the main Brooks Rehabilitation campus on Beach Blvd. near University Blvd. in Jacksonville, Florida. This three-story, 82,000 sq. ft. facility has 111 beds and serves patients in need of both short and long-term rehabilitation.

Each of its three floors houses a therapy gym, living room, activity space and dining room. The facility also features walking paths, a healing garden and an outdoor therapy courtyard providing a peaceful setting for patients and their families.
